Use this checklist to organize records before consultation and insurer-facing discussions. Because coverage can be difficult to secure, complete documentation and follow-up notes are often essential. You can save or print this page and mark items as you gather them.

1. Patient and Coverage Basics

[ ] Insurance card (front/back) and policy details.
[ ] Primary contact information and insurer portal access.
[ ] Referral requirements or PCP coordination notes.

2. Medical Records Packet

[ ] Symptom timeline (pain, tenderness, heaviness, bruising).
[ ] Prior diagnosis notes and specialist evaluations.
[ ] Relevant imaging or supporting reports if available.
[ ] Medication and relevant comorbidity history.

3. Conservative Care History

[ ] Compression and lymphatic therapy history.
[ ] Exercise, nutritional, and weight-management attempts.
[ ] Duration of each conservative modality and outcomes.

4. Functional Impact Summary

[ ] Daily activity limitations (walking, standing, work tasks).
[ ] Mobility or endurance changes over time.
[ ] Quality-of-life impact statement in your own words.

5. Submission and Follow-Up Tracker

[ ] Date records were submitted to insurer.
[ ] Reference number / case number.
[ ] Follow-up call dates and representative names.
[ ] Appeals or additional-document request log.
